Imminent Safety Risk:
1. Stay with client or keep on the phone
2. Call 911 if immediate danger
3. Contact on-call supervisor immediately
4. Document in clinical notes

Crisis Assessment:
1. Assess suicidal/homicidal ideation, plan, and means
2. Involve supervisor for any active ideation
3. Consider voluntary or involuntary hospitalization
4. Contact family/supports with appropriate consent

After a Crisis:
1. Complete incident report within 24 hours
2. Debrief with supervisor
3. Schedule follow-up with client
4. Review safety plan
